I failed to understand how to add Album and Disk sub-headers (?)
The Group Header I found, but not the other two...
(By the way; what's the difference between an Album and a Disk ?)

They are on the grid, you can make them visible via the "Select Columns" link in the grid footer:

Enable filter on going to db view from hierarchical.
Track group Header can be split into two levels with separator (::) - see wiki.
